A New Complication Associated with the Use of Prophylactic Intracameral Antibiotics: Hemorrhagic Occlusive Retinal Vasculitis.

چکیده
Given the substantial increase in antimicrobial resistance among ocular pathogens worldwide, evidence-based strategies are needed to carefully analyze the risks and benefits of routine prophylactic intracameral antibiotics for prevention of infection in cataract surgery. In ophthalmology, 2 basic issues are debated frequently: (1) the effectiveness of intracameral antibiotics in lowering rates of endophthalmitis in association with cataract surgery and (2) the use of vancomycin as a prophylactic antibiotic. Intracameral antibiotics during cataract surgery, although used in some parts of the world, remain controversial. Prophylactic antibiotics are associated with increased costs, risks to the individual patient, and risks to the population at large, by contributing to the emergence of drug-resistant or-

Early anti-VEGF treatment for hemorrhagic occlusive retinal vasculitis as a complication of cataract surgery
BACKGROUND We report a case of hemorrhagic occlusive retinal vasculitis (HORV) after prophylactic intracameral vancomycin use during an uneventful cataract surgery treated with early anti-VEGF treatment. CASE PRESENTATION A 51-year-old female underwent uneventful cataract surgery with prophylactic intracameral vancomycin during the procedure. On the seventh post-operative-day, she presented w...
متن کاملIntracameral antibiotic: the Vancomycin conundrum.
Delayed complications of cataract surgery are uncommon. Where previously endophthalmitis was the most feared complication now there is a new contenderHaemorrhagic Occlusive Retinal Vasculitis (HORV). These two complications can present a week after surgery and result in no perception of light. Vancomycin has been used extensively as a prophylaxis against endopthalmitis, and is now thought to be...
متن کاملPostoperative endophthalmitis
Postoperative endophthalmitis is a rare, albeit serious, complication of cataract surgery. Over the years, preoperative and operative measures, such as lid hygiene, appropriate surgical draping, and improved surgical technique, have all decreased the incidence of postoperative endophthalmitis. Commonly used prophylactic measures include preoperative topical, intracameral, and postoperative topi...
متن کاملRole of intravitreal/intracameral antibiotics to prevent traumatic endophthalmitis – Meta-analysis
Traumatic endophthalmitis is a devastating condition that can occur following an open globe injury and result in loss of vision. The use of prophylactic antibiotics is empirical as most surgeons fear complications associated with the same. No systematic review has been performed in English on the role of intravitreal/intracameral antibiotics in preventing traumatic endophthalmitis. We searched ...
